Credit Card Application Requirements

Eligibility Requirements. To be eligible for an Equicom Visa Credit Card, you MUST:

  • Be between 21 to 70 years old
  • Be a Philippine resident
  • Have at least gross annual income of Php120,000 for Classic or Php500,000 for Gold (income must be verifiable and consistent with documents submitted)

Documentary requirements

  1. Duly accomplished and signed application form
  2. Proof of Income. Under BSP Circular 622, Applicant MUST be able to submit their latest Income Tax Return with BIR/Bank Stamp or BIR Form 2316 AND

    Employed Applicants, submit ANY of the following and must not be older than three (3) months

    • Certificate of Employment stating hire date, position and compensation
    • Employment Contract
    • Letter of Appointment
    • Notice of Salary Increase
    • Latest Payslip/s showing one month income
    • Latest Credit Card Statement

    For Self-Employed applicants (MUST submit all)

    • Tax Return (most recent and full-year)
    • Audited Financial statements
    • Copy of registration with SEC, DTI or the appropriate government agency
    • Latest Credit Card statement or certificate of bank deposit, if business is not
      among the top 5,000 corporations

    For Foreigners, please submit a photocopy of ANY of the following

    • Philippine Visa
    • Alien Certificate of Registration stating the type of Visa held
    • Immigration Certificate of Residence (ICR)
      Note: The documents must have a validity of at least 1 year from the date
      of application
  3. Proof of Identification. Applicant MUST submit two (2) valid IDs (front and back copies) of any of the following:
    • Passport
    • Driver’s License
    • Company ID
    • SSS
    • GSIS Photocard
    • Philippine Regulatory Commission (PRC) ID
    • BIR ID
    • Integrated Bar of the Philippines ID
    • ACR/ICR-Immigrant Certificate of Registration
    • Government Office and GOCC ID
    • OFW/OWWA ID (card type)

Note: All data submitted will be treated confidentially by Equicom Savings Bank. Equicom Savings Bank will only process applications upon receipt of all required documents and data, and documents supplied will not be returned. Equicom Savings Bank has the right to decline your credit card application, without having to specify and/or disclose the reason.
