Eligibility Criteria For Company Accreditation

  • Registered with the Securities and Exchange Commission (SEC), Department of Trade and Industry (DTI), Cooperative Development Authority (CDA), or other relevant government agencies.
  • Must have at least 30 permanent employees
  • Not engaged in businesses that are classified as high risk by Equicom Savings Bank

Pre-Accreditation Requirements

  • Company Profile
  • Latest General Information Sheet (GIS)
  • SEC Registration and Articles of Incorporations and By-Laws
  • Latest Audited Financial Statements (AFS) for the past three (3) years
  • Corporate Income Tax Return (BIR Form 1701) for the past three (3) years

Post-Accreditation Requirements

  • Memorandum of Agreement
  • Secretary Certificate
  • Photocopy of two (2) valid IDs of the company’s authorized signatories with three (3) specimen signatures
